Building Pathways Toward Stability, Growth & Wellness
Trauma-informed, community-based support for youth, adults, families and the organizations that serve them.
Bridging individuals and families with resources.
Horizon Pathways Wellness LLC was founded with the belief that individuals and families should not have to navigate difficult life circumstances and complex community systems alone. Horizon was created to bridge the gap between individuals, families, and the resources available within their communities.
Through trauma-informed support, resource navigation, family engagement, and community partnerships, Horizon works to help people identify practical pathways toward greater stability, independence, and wellness.
